In an effort to better manage your health, Hawthorne Clinic encourages all eligible patients over 75 years to undertake a bulk billed Annual Health Assessment.
WHY HAVE AN ANNUAL HEALTH ASSESSMENT?
Taking an Annual Health Assessment gives your GP an opportunity to take a step back and look at your total health with a view to improving your current health status and medical conditions, along with looking to prevent any problems in the future.
WHAT IS HAPPENS IN AN ANNUAL HEALTH ASSESSMENT?
As part of your Annual Health Assessment, one of our friendly registered nurses will guide you through a series of questions about your current health, medications, activities of daily living and what support services you currently use.
At this time we will take some basic measurements such as your weight, height and blood pressure.
The Health Assessment will take approximately 20 minutes.
An appointment with your GP will then follow to complete the Health Assessment. Recommendations will be made as to how you can improve your health and wellbeing.

To book your assessment contact our reception team on 3399 5444 to request an Over 75's Health Assessment with your nurse and GP.
If you have any questions prior to booking you may ask to speak with the Practice Nurse.

Peak’s custom classes identify fall risks, improve strength and balance and are designed to help you regain your independence.
